This is a space where document templates created directly by users are stored. You can save frequently used consent forms, contracts, and questionnaires like templates and reuse them repeatedly. Authors can edit, duplicate, or delete templates at any time and filter them by type. Template types are broadly divided into 3 categories:
E-Document: All documents that do not fall under the Intake Form or Log types are classified as E-Documents. (Example: Consent forms, Agreement on Collection and Use of Personal Information and Provision to Third Parties, etc.) You can enter text directly, and the use of basic input fields and auto-fill information widgets is supported.
Intake Form: A document used to create questionnaires that collect customer information. (Example: Intake interview forms, consultation application forms, etc.) You can build these freely based on the standard question widgets provided, and some question widgets automatically link and sync to the customer's profile information upon entry.
Auto-Sync Items: Name, Date of Birth, Gender, Contact Number, Email, Address, Occupation, Notes
Lote: A document used to record the content conducted during a specific session either after a service ends or while a video service is in progress. (Example: Session records, consultation logs, etc.) Items such as Customer Name, Session Information, Session Goals, and Content are built into a log-type template using widgets. Only one log can be written for each service schedule (session), and you can create and use different formats of logs for each service provided.
